Pathways to Early Learning Development Transition Support

About the program

Transition Support Workers help ensure you and your child are supported throughout the many transitions which occur from 0-8 years old.

We can support school readiness and attendance, and health & well-being of your child where needed by:

  • Assisting with paperwork in relation to your child care or education service
  • Linking you with family support services available in the community
  • Referrals to health and well-being services
  • Programs and activities which improve educational outcomes
  • Assisting with and providing information on routines, healthy eating, health, hygiene and child development.
